We are seeking an experienced Field Care Manager to lead our team in delivering exceptional care in the community.

About the Role

The successful candidate will be responsible for overseeing the delivery of high-quality home care services in the North West region. This will involve conducting client assessments, managing care plans, and ensuring compliance with CQC regulations.

About Our Company

Libertatem Healthcare Group is a registered provider with the Care Quality Commission (CQC) and the Care Inspectorate Wales (CIW). We have a national presence and can provide care and support to clients wherever they are located.

Key Responsibilities:

  • Conducting client assessments and managing care plans.
  • Ensuring compliance with CQC regulations.
  • Liaising with the operational team regarding performance issues and training needs.
  • Building relationships with clients and their families.

Requirements

  • NVQ Level 3 or 4 in Health & Social Care (or equivalent).
  • Experience in a supervisory/management role within health and social care.
  • A passion for delivering high-quality care, exceptional communication, and organizational skills.
  • Full UK driving license and ability to travel using own car.

What We Offer:

  • A competitive salary of £27,000 depending on skills and experience.
  • Remote/field-based role.
  • Working hours – 9am-5:30 pm Monday to Friday with some flexibility over weekends/evenings.
  • Annual leave - 25 days plus bank holidays.
  • Ongoing training.
