Job Summary

The Forward Trust is seeking a highly motivated Substance Misuse Family Support Coordinator to join our team in Buckinghamshire. This role offers a unique opportunity to make a real difference in the lives of individuals and families affected by substance misuse.

About the Role

As a Substance Misuse Family Support Coordinator, you will work closely with clients and their families to provide specialist support and guidance. Your primary focus will be on helping clients identify and address the root causes of their addiction, as well as promoting healthy relationships and behaviors.

You will also work directly with children and young people to promote whole family recovery and rehabilitation. This may involve developing and implementing individualized support plans, conducting needs and risk assessments, and facilitating group interventions.

The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will be required to travel within the local area and may need to work irregular hours.

Responsibilities
  • Develop and implement individualized support plans for clients and their families
  • Conduct needs and risk assessments to identify areas for improvement
  • Facilitate group interventions and family meetings
  • Collaborate with other organizations to provide comprehensive support services
Requirements

What We Offer
