**Job Summary:**

We are seeking an experienced and skilled Community Nurse to join our team at NHS Professionals in Warrington. As a key member of our community nursing team, you will be responsible for providing high-quality patient care and supporting the District Nursing team in delivering effective service delivery.

**Key Responsibilities:**

  1. Promote health and wellbeing within the family unit and community as a whole.
  2. Perform comprehensive assessments of patient nursing needs as the named nurse.
  3. Plan, assess, and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
  4. Support the team leader in the co-ordination of complex and continuing care packages.
  5. Communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
  6. Be professionally responsible for a delegated caseload under the supervision of the District Nurse.

**Required Skills and Qualifications:**

  • A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years.
  • Experience with Wound Care, insulin and medication administration, catheter care, and palliative care.
  • NMC Professional Registration.
