Job Overview:

We are looking for a highly skilled Community Health Practitioner to join our team at the East London NHS Foundation Trust. As an Occupational Therapy Specialist, you will be responsible for providing a specialist occupational therapy service to patients in their own homes.

The ideal candidate will have an Occupational Therapy Degree and at least 18 months of post-registration clinical practice working in a rehab setting. You will also have experience working as part of a multidisciplinary team and be able to work independently with minimal supervision.

The role involves assessing and managing complex cases, developing specialist skills in relation to specific client groups, and managing a caseload of complex cases. You will also provide verbal and written information to other multidisciplinary professionals regarding the occupational therapy assessment, management, and treatment of the client.

In addition, you will participate in client-related meetings as required and communicate effectively with medical, nursing, and therapy colleagues, as well as outside agencies, to ensure the delivery of a coordinated multidisciplinary service.
