Mental Health Practitioner

We are seeking a dedicated and passionate Mental Health Practitioner to join our team at Central and North West London NHS Foundation Trust. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care and support to individuals experiencing mental health crises in the Brent area.
The successful candidate will be part of a friendly and supportive team, committed to delivering NICE-concordant care and maximizing autonomy for service users. You will participate in the team's duty and assessment systems, working collaboratively with other members of the multi-disciplinary team to ensure seamless care pathways.
Key responsibilities include undertaking initial and comprehensive assessments, care coordination, and co-worker responsibilities for a designated group of service users. You will also provide supervision to junior colleagues and students, as well as develop and maintain effective communication strategies between Mental Health Services and General Practice Teams.
Main Duties and Responsibilities- Undertake initial and comprehensive assessments of health and social care needs, working alone or with colleagues, and provide feedback to the Multi-Disciplinary Team.
- Provide care coordination and co-worker responsibilities for a designated group of service users, ensuring a recovery-focused and socially inclusive approach.
- Supervise junior colleagues and students, providing guidance and support to ensure high-quality care and professional development.
- Develop and maintain effective communication strategies between Mental Health Services and General Practice Teams, improving care pathways and outcomes.
- Work within CNWL policies and procedures, upholding the values and ethos of the organization, and adhering to professional and legal frameworks.
- Significant experience of working in a clinical area, providing mental and physical health care and assessment.
- Demonstrable experience of working with individuals who experience serious ongoing mental health problems.
- Knowledge and experience of working with people prescribed Clozapine or Long Acting Injection (LAI) medication, medication effects, side effects, and patient management.
- Experience of working with people in the community, with a focus on recovery and social inclusion.
- Qualified Mental Health Nurse or OT, with current registration with the relevant professional body.
- Non-medical prescribing, with a focus on medication management and patient safety.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with other healthcare professionals and service users.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands.
