Primary Care Mental Health Nurse

The successful candidate will be part of a dynamic team of clinicians responsible for developing and improving services for individuals with common mental health problems in primary care.
Main Duties- Provide effective consultation and liaison across primary and secondary care sectors
- Facilitate and improve access to appropriate services
- Conduct mental health screening assessments, risk assessments, and medication monitoring
- Offer telephone triage, signposting, and referrals to other services
- Provide brief interventions for individuals experiencing common mental health problems
- Support and educate local primary care teams on mental health assessment and management
- Undertake home visits and work across different geographical settings
- Participate in audit and research activities
- Develop services to meet the mental health needs of users and carers in primary care
- Registered Nurse Mental Health with current NMC registration
- Post-registration experience in community mental health services
- Experience working collaboratively within a multi-disciplinary team
- Excellent communication and organizational skills
- Training in teaching, assessing, and clinical supervision
- Experience in management or leadership
- Ability to mentor student nurses
