Primary Care Mental Health Nurse

The post holder will be part of a dynamic team of clinicians responsible for developing and improving services for individuals with common mental health problems in primary care. The role involves working across designated GP practices within primary care networks, attending weekly integrated care team meetings.
Main DutiesThe post holder will provide effective consultation and liaison across primary and secondary care sectors, facilitating access to appropriate services. Key responsibilities include mental health screening assessments, risk assessment, medication monitoring, telephone triage, signposting, referral to other services, and brief intervention for individuals experiencing common mental health problems.
The post holder will also provide support, education, and problem-solving advice to local primary care teams, particularly in areas of assessment and management of mental health problems. This will involve working across different geographical settings and undertaking home visits.
Key Requirements- Entry on a Register held by the Nursing and Midwifery Council (Sub Part 1 RN3, Registered Nurse Mental Health)
- Evidence of current continued professional development
- Willingness to undertake non-medical prescribing
- Post-registration experience of working autonomously in community mental health services
- Experience of working collaboratively within a multi-disciplinary/multi-agency team
- Experience of managing and organising own workload
- Skills in assessing and formulating individual intervention plans and outcome measures for adults with mental health problems
- Risk assessment/risk management skills
- Knowledge of latest evidence-based practice and NICE guidance for supporting individuals with mental health needs
- Knowledge of the principles of clinical governance/safeguarding and its application
- Excellent communication skills, oral and written
- Car Driver
- Training relevant to the role - teaching and assessing, clinical supervision, motivational interviewing, solution-focused working
- Supporting staff in the day-to-day provision of clinical services
- Experience of management or leadership
- Experience of delivering new and innovative practice in a clinical setting
- Mentorship of student nurses
