Primary Care Mental Health Nurse

The post holder will be part of a dynamic team of clinicians responsible for developing and improving services for individuals with common mental health problems in primary care. The role involves working across designated GP practices within primary care networks, attending weekly integrated care team meetings.
Main DutiesThe post holder will provide effective consultation and liaison across primary and secondary care sectors, facilitating and improving access to appropriate services. Key responsibilities include:
- Providing mental health screening assessments, risk assessments, medication monitoring, telephone triage, signposting, and referrals to other services.
- Supporting and educating local primary care teams on assessment and management of mental health problems.
- Undertaking home visits and working across different geographical settings.
The ideal candidate will possess:
- Registration on the Nursing and Midwifery Council (NMC) register (Sub Part 1 RN3, Registered Nurse Mental Health).
- Evidence of current continued professional development.
- Willingness to undertake non-medical prescribing.
- Post-registration experience working autonomously in community mental health services.
- Experience working collaboratively within a multi-disciplinary/multi-agency team.
- Skills in assessing and formulating individual intervention plans and outcome measures for adults with mental health problems.
- Risk assessment/risk management skills.
- Knowledge of latest evidence-based practice and NICE guidance for supporting individuals with mental health needs.
- Excellent communication skills (oral and written).
The following skills and experience are desirable:
- Training relevant to the role (teaching and assessing, clinical supervision, motivational interviewing, solution-focused working).
- Supporting staff in the day-to-day provision of clinical services.
- Experience of management or leadership.
- Experience of delivering new and innovative practice in a clinical setting.
- Mentorship of student nurses.
