Elysium Healthcare is seeking a dedicated Care Coordinator to join our team at Rosebank House, a specialist care home for individuals with severe and enduring mental health conditions.

As a Care Coordinator, you will play a vital role in creating a supportive and engaging environment for our service users. Your primary responsibility will be to plan, coordinate, and deliver social and recreational activities that promote social inclusion, education, and employment engagement.

Key Responsibilities:

  • Develop and implement activity plans that cater to the diverse needs and interests of our service users.
  • Build strong relationships with service users, their families, and the multidisciplinary team to ensure seamless care coordination.
  • Collaborate with community-based resources to provide access to local activities and services.
  • Monitor and evaluate the effectiveness of activity plans, making adjustments as needed.
  • Contribute to a culture of continuous learning and professional development within the team.

Requirements:

  • Minimum 12 months experience in a mental health setting (desirable).
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-based framework.
  • Initiative and motivation to drive positive change.
  • Awareness of mental health and its impact on functioning.
