Support Needs Assessor

We are seeking an experienced Social Worker to join our Adult Social Care team at PP Associates Ltd. The successful candidate will be responsible for assessing, planning, and implementing support for adults with varying needs, ensuring they receive the highest quality of care and support.
Responsibilities:
• Conduct comprehensive assessments of individuals' needs, including their physical, emotional, and social well-being;
• Develop and implement personalized care plans that cater to the unique requirements of each service user;
• Collaborate with other healthcare professionals, family members, and carers to ensure effective coordination of care;
Requirements:
To succeed in this role, you will need;
• A degree in Social Work or a related field;
• Significant experience working in Adult Social Care, preferably in a leadership or management capacity;
• Exceptional communication and interpersonal skills;
• Ability to work autonomously and as part of a team;
• Strong assessment and planning skills;
• Familiarity with relevant laws and regulations;
