We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group. As a Support Coordinator, you will play a vital role in ensuring our customers receive the necessary support to develop their independence and achieve their desired outcomes.

Key Responsibilities
  • Work under the supervision of your Senior Client Service Manager and Senior Support Coordinator to ensure our customers receive the necessary support.
  • Support customers with varied support needs, including mental health issues, housing-related support, and complex needs.
  • Create and update support plans and risk assessments with customers, helping them with daily living activities such as making meals, shopping, and budgeting.
  • Review support plans and risk assessments to ensure customers' needs are being met.
  • Maintain our properties, ensuring they are clean, homely, and well-maintained.
  • Collaborate with colleagues and participate in cuppas, chats, and catch-ups.
Requirements
  • Experience of creating person-centred support plans, risk assessments, and reviews.
  • Experience of coordinating and assessing customer referrals.
  • Ability to work on your own initiative, remain calm under pressure, and have a resilient approach.
  • Understanding of customers' needs and the passion to advocate for them.
  • Experience of delivering support in a housing or care environment.
  • A vehicle insured for business purposes and willingness to travel.
