About the Role

We are seeking a dedicated and compassionate Support Coordinator to join our team at Home Group. As a Support Coordinator, you will play a vital role in ensuring our customers receive the necessary support to develop their independence and achieve their desired outcomes.

Key Responsibilities

  • Support customers with varied support needs, including mental health issues, housing-related support, and complex needs.
  • Create and update support plans and risk assessments with customers, helping them with daily living activities such as making meals, shopping, budgeting, and maintaining their tenancy.
  • Review support plans and risk assessments to ensure customers' needs are being met.
  • Ensure our properties are well maintained, clean, and homely, promoting a positive living environment for our customers.
  • Collaborate with colleagues to provide a supportive and inclusive team environment.

Requirements

  • Experience of creating person-centred support plans, risk assessments, and reviews.
  • Ability to work on your own initiative, remain calm under pressure, and have a resilient approach.
  • Understanding of customers' needs and the ability to advocate for them.
  • Basic knowledge of Microsoft Word and Excel, as well as the ability to use technology for creating and updating support plans.
