Job Title: Mental Health Support Worker

Service Care Solutions is seeking a dedicated and compassionate Mental Health Support Worker to join our team at one of our Independent Hospitals in Barnstaple.

Job Purpose:

To provide high-quality support and care to individuals accessing our services, ensuring their physical and mental wellbeing is prioritized.

Key Responsibilities:
  • Deliver emotional support, advice, and guidance to individuals via face-to-face, telephone, and video calls.
  • Assess situations to determine risk and individual needs, making informed decisions under pressure.
  • Accurately record new contacts and details of visits and calls on relevant electronic systems.
  • Work in line with performance targets and key contractual requirements, providing data and case studies as requested.
  • Advise and be actively involved in individual risk assessment and review, developing appropriate action plans and safety plans as needed.
  • Encourage individuals to recognize, understand, and manage factors affecting their mental wellbeing and physical health.
  • Collaborate with a range of partners and external agencies to meet individual needs and support with individual safety and delivery of the service.
  • Work as a team member, sharing information and supporting colleagues to ensure they have the knowledge and information required to deliver high-quality care.
