About the Role

We are seeking a skilled Clinical Social Worker to join our Adult Social Care team. As a key member of our team, you will play a vital role in supporting vulnerable adults in Essex, working closely with them, their families, and care providers to develop and implement personalized care plans.

Key Responsibilities

  • Conduct comprehensive assessments and care reviews to identify individual needs and develop effective care plans.
  • Collaborate with internal and external partners, including health services and community organizations, to ensure holistic support and coordination of care.
  • Work closely with adults with complex needs, including those related to learning disability, autism, mental or physical health, to empower them to achieve independence and well-being.

Requirements

  • Qualified Social Worker with a current Social Work England registration.
  • At least 1 year's post-qualifying social work experience, with evidence of continuing professional development and capability of practice in accordance with current professional standards.
  • Strong understanding of the Care Act, Mental Health Act, Mental Capacity Act, and Deprivation of Liberty and Court of Protection procedures.
