Job Overview:

We are seeking a highly skilled and motivated Social Worker to join our team, providing enhanced social care support for individuals with learning disabilities and autism.

About the Role:

This is an exciting opportunity to work in a dynamic and innovative team, delivering person-centred care and promoting independence through a positive approach to risk. As a Social Worker, you will be responsible for undertaking Care Act reviews, developing support plans, and working closely with adults, their families, and care providers to achieve their goals.

Key Responsibilities:

  • Conduct thorough assessments and develop tailored support plans
  • Work collaboratively with multidisciplinary teams, including health professionals and care providers
  • Provide guidance and support to individuals, their families, and carers
  • Manage caseloads and prioritize tasks effectively

Requirements:

  • Qualification in Social Work (DipSW or CQSW) and registration with Social Work England
  • At least 1 year's post-qualifying experience in social work, preferably in a similar field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
