Job Summary:

We are seeking an experienced Rehabilitation Case Manager to join our team at Active Care Group. As a Rehabilitation Case Manager, you will be responsible for providing high-quality care and support to individuals with ongoing needs, helping them to improve their quality of life and maximize their potential.

Key Responsibilities:

  • Advocate for clients' needs while building strong working relationships with families, deputies, and litigation solicitors
  • Develop and implement individualized care and support plans, including action plans and goals with specific timeframes
  • Recruit, select, and manage a team of care/support workers for clients
  • Implement and monitor case management plans and support/rehabilitation plans, making adjustments as needed
  • Provide flexible working patterns to support teams, ensuring smooth service operation, including out-of-hours, weekends, and public holidays
  • Compile risk assessments and identify risk management strategies to ensure safety for all parties

Requirements:

  • Hold a UK professional health qualification requiring registration with HCPC, NMC, or Social Worker
  • Full driving license and access to a car
  • Previous experience as a Case Manager, preferably supporting adults or children, with a full induction and training provided
  • Thorough knowledge of CQC fundamental standards and key lines of enquiry
  • Working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and Safeguarding frameworks
  • Ability to address and respond to employee issues in a fair and consistent manner
  • Strong organizational, time management, and planning skills
  • Flexible, professional, and positive approach to multi-site working
