Job Overview

Voyage Care is seeking a highly skilled Support Planning Coordinator to join our team. As a key member of our team, you will be responsible for developing and implementing individual support plans that meet the unique needs of each person supported.

Key Responsibilities:
  • Support Planning: Develop and implement individual support plans that meet the needs of each person supported.
  • Caseload Management: Manage a caseload of individuals, ensuring their needs are met and their lives are enhanced.
  • Staff Leadership: Lead and manage staff teams, providing guidance and support to ensure excellent care delivery.
  • Compliance: Maintain compliance with company policies and procedures, including administration of medication and safeguarding.

Requirements:

  • Minimum 1 year experience in care management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic IT skills and ability to use software applications.
  • NVQ Level 3 or equivalent qualification in care or a related field.

Salary and Benefits:

The salary for this role is estimated at £28,000-£35,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including annual leave, sick pay, and opportunities for professional development.
