**Job Title:** Senior Diabetes Specialist Nurse
We are seeking a highly skilled and experienced Senior Diabetes Specialist Nurse to join our team at Lincolnshire Community Health Service NHS Trust.
Job Description:
The successful candidate will be responsible for providing specialist clinical management to patients with diabetes, utilizing advanced skills and knowledge to facilitate effective patient care.
Key Responsibilities:
- Lead the provision of diabetes care for patients in Lincolnshire, including clinical care, education, and training for patients and staff.
- Provide advanced clinical care, using high-level assessment to make diagnoses, initiate treatments, refer appropriately, and coordinate patient journeys across healthcare systems.
- Improve clinical outcomes for patients by enabling them to function independently, reducing hospital admissions, and promoting community-based care.
- Manage a delegated budget to support specialist functions within the Diabetes service.
- Meet performance and target levels as identified by LCHS and national diversions.
- Identify high-intensity users with complex needs using risk stratification systems and other sources of information.
- Support the Diabetes team to meet quality and performance targets.
- Take a leadership role in the continuous development of the service.
Requirements:
- First-level adult registration with the NMC.
- Non-medical Prescriber or willingness to work towards it.
- Masters-level qualification or equivalent experience and knowledge.
- In-depth understanding of diabetes management.
- Advanced clinical assessment skills.
- Able to prioritize workload to address patient urgent needs.
- Flexible approach to meet service needs, self-motivated, and enthusiastic about the role.
