**Lincolnshire Community Health Service NHS Trust** is seeking a highly skilled and experienced Senior Diabetes Specialist Nurse to join our team.

Job Description:

The successful candidate will be responsible for providing specialist clinical management to patients with diabetes, utilizing advanced skills and knowledge to facilitate effective patient care.

Key Responsibilities:

  • Lead the provision of diabetes care for patients in Lincolnshire, including clinical care, education, and training for patients and staff.
  • Provide advanced clinical care, using high-level assessment to make diagnoses, initiate treatments, refer appropriately, and coordinate patient journeys across healthcare systems.
  • Improve clinical outcomes for patients by enabling them to function independently, reducing hospital admissions, and promoting community-based care.
  • Manage a delegated budget to support specialist functions within the Diabetes service.
  • Meet performance and target levels as identified by LCHS and national diversions.
  • Identify high-intensity users with complex needs using risk stratification systems and other sources of information.
  • Support the Diabetes team to meet quality and performance targets.
  • Take a leadership role in the continuous development of the service.

Requirements:

  • First-level adult registration with the NMC.
  • Non-medical Prescriber or willingness to work towards it.
  • Masters-level qualification or equivalent experience and knowledge.
  • In-depth understanding of diabetes management.
  • Advanced clinical assessment skills.
  • Able to prioritize workload to address patient urgent needs.
