Advanced Community Nursing Professional

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Midlands Partnership NHS Foundation Trust.
Main Job Responsibilities:- To provide high-quality care to patients in their own homes, focusing on holistic needs assessment and treatment planning.
- To work collaboratively with the multi-disciplinary team to promote patient-centered care and support independence.
In this role, you will be responsible for managing a defined caseload of patients, delivering care directly to those who require it. You will also work autonomously, utilizing your scope of practice and policies to ensure excellent patient outcomes.
Key Skills and Qualifications:
- A degree in Nursing or equivalent qualification.
- NMC Registration as a Registered Nurse (Adult) is essential.
- A commitment to ongoing professional development, particularly in areas such as palliative care, wound management, and medication administration.
