Home-Based Nursing Professional

Midlands Partnership NHS Foundation Trust is seeking a skilled and experienced Community Staff Nurse to join our District Nursing Team. As a key member of our team, you will play a vital role in delivering high-quality care to patients in their own homes, promoting independence, and supporting patient-centered care planning.
Responsibilities- Deliver care to patients in their own homes or residential homes, and/or in clinic settings where appropriate.
- Assess and manage patient care needs, developing and implementing care plans, and evaluating patient outcomes.
- Work collaboratively with other healthcare professionals to promote patient-centered care planning and support.
You will be responsible for assessing and managing patient care needs, using your advanced clinical skills and judgment to develop and implement effective care plans. You will also work autonomously, using your scope of practice, competence, and policies to make decisions and take action.
What We OfferThis role offers a competitive salary of £34,880 - £44,606 per annum, depending on experience.
