We are seeking an experienced and skilled Community Staff Nurse to join our District Nursing Team within Mercian PCN as a Band 5 Community Staff Nurse.

About the Role

The successful candidate will have a commitment to providing holistic care to patients in their own homes, working autonomously and collaboratively with the multi-disciplinary team to promote independence and support patient-centred care planning.

Key responsibilities include delivering care to patients in various settings, assessing and managing long-term conditions, palliative care conditions, and wound care. The ideal candidate will have relevant experience and placements in surgical, medical, and palliative care.
