Job Overview

We are seeking a highly skilled and motivated Community Staff Nurse to join our District Nursing Team at Midlands Partnership NHS Foundation Trust. This is an exciting opportunity for a passionate and innovative nurse to deliver high-quality care to patients in their own homes.

About the Role

As a Community Staff Nurse, you will be responsible for providing holistic care to patients with a range of health needs, including palliative care, complex wound care, pelvic health, and medication administration. You will work autonomously, using your advanced clinical skills and judgment to assess and manage patient care needs.

You will be part of a dynamic and supportive team, working collaboratively with other healthcare professionals to promote independence and support patient-centered care planning. The ideal candidate will have excellent communication and organizational skills, with a commitment to delivering high-quality care in line with evidence-based practice.

Key Responsibilities
  • Deliver care to patients in their own homes or residential homes, and/or in clinic settings where appropriate.
  • Be responsible for assessing and managing patient care needs, developing and implementing care plans, and evaluating patient outcomes.
  • Work autonomously, using your scope of practice, competence, and policies to make decisions and take action.
Person Specification

The ideal candidate will have a degree in Nursing or equivalent, NMC registration, and a commitment to work towards mentorship qualification (998-997). They will also have relevant experience and placements in assessment and management of long-term conditions, palliative care, and wound care. A full driving license is essential, as travel across the patch is required to provide care to patients.

Salary and Benefits

This role offers a competitive salary of £34,880 - £44,606 per annum, depending on experience.
