We are seeking an exceptional Advanced Healthcare Specialist to join our team at Midlands Partnership NHS Foundation Trust, driving excellence in patient care through advanced clinical practice and non-medical prescribing. Our innovative Community Rapid Intervention Service (CRIS) and General Acute Medicine and Frailty Virtual Ward bring together the expertise of University Hospital of North Midlands, University Hospital of Derby and Burton, and Midlands Partnership Foundation Trust to deliver compassionate and effective care.
In this critical role, you will work collaboratively with multidisciplinary teams to develop and implement best-practice care pathways, ensuring seamless transitions between hospital and community settings. Opportunities for career progression from band 6 to band 7 allow you to grow professionally and personally while making a meaningful difference in patient lives. Key responsibilities include leading by example, mentoring junior colleagues, and contributing to quality improvement initiatives.
