About the Role

This exciting opportunity is for a highly skilled Advanced Community Practitioner to join our team at Midlands Partnership NHS Foundation Trust. As an integral part of our Community Rapid Intervention Service (CRIS), you will play a vital role in delivering acute care to patients in their own homes.

The post holder will be a Registered Healthcare Professional and hold an MSc in Advanced Clinical Practice or equivalent. They will practice at an advanced level, demonstrating in-depth knowledge and competence in all aspects of acute care within the patient's home setting & environment.

Responsibilities
  • Communicating sensitive information to patients and relatives, providing empathy and reassurance.
  • Collaborating with the healthcare team to inform decision-making processes.
  • Leading on complex cases, requiring analysis, interpretation, and comparison of options.

We are looking for someone who is passionate about delivering high-quality patient-centred care and has a strong clinical background in emergency/acute/general medicine care. The ideal candidate will have experience of teaching, supervising, and mentoring learners, as well as leadership skills.
