Risk Management Practitioner

We are seeking an experienced and skilled Risk in the Community Senior Practitioner to join our team at Pennine Care NHS Foundation Trust.
About the Role:
This is a dynamic role that requires the development of specialist skills to deliver recovery-focused interventions to adults in a mental health crisis. As a Risk Management Practitioner, you will be responsible for making clear decisions about risk management, care planning, and the appropriateness of home treatment team interventions.
Your Key Responsibilities:
- Assessing service users' needs and providing intensive home treatment in their homes/community settings.
- Ensuring compliance with safeguarding children and the Protection of Vulnerable Adult Policy and statutory obligations.
- Evaluating systematically and recording outcomes of mental health assessments, adapting care on the basis of these outcomes.
- Collaborating with family and carers in the assessment and care planning process with service users' consent.
- Developing and implementing education and self-help resources for patients and carers tailored to their needs.

Education / Qualifications:
Essential criteria: RMN/Social Worker/OT.
Experience:
Essential criteria: Extensive knowledge of mental health legislation and guidance.
Knowledge:
Essential criteria: Knowledge and experience of working within a multidisciplinary team.
Salary: £40,000 - £60,000 per annum.
