About The Role
As a Supportive Housing Facilitator, you will deliver a tailored, structured resettlement service to each individual at our facility. Your primary goal will be to assist individuals who have faced homelessness in transitioning to their own residences in a thoughtful and supported manner.
In this role, you will provide guidance, training, information, and both individual and group support to navigate this journey. The assistance offered will be personalized, encompassing advice on housing rights, accompanying individuals to important meetings, and practical support for moving in, such as securing furniture and establishing payment plans for utilities.
It is essential to ensure that individuals ready to transition are equipped with the necessary support to enhance their independence, dignity, and choices, aligning with the project's objectives.
Key Responsibilities:
  • Deliver information on housing alternatives, assist with accommodation applications, advocate for clients with housing providers, and accompany them to viewings and interviews as needed.
  • Serve as a key-worker for a designated caseload, empowering clients to take charge of their journey.
  • Ensure clients receive essential practical assistance for moving in, either directly or through collaboration with other support services, including personal and housing benefit claims, community care grants, and utility connections.
  • Conduct a strengths-based assessment with applicants, facilitating the development of a personalized plan that helps individuals achieve their goals and identify their strengths.
  • Build and maintain effective relationships with specialists and agencies that can provide resources, funding, or services relevant to client needs, such as social landlords, local authorities, and community services.

  • Experience or knowledge in working with homeless individuals.
