The Supportive Housing Facilitator will deliver a tailored, structured resettlement service to each individual at SAHA. The primary goal is to assist individuals who have faced homelessness in transitioning to their own residences in a planned and supportive manner.
To provide guidance, training, information, and both individual and group support to navigate this journey. The assistance offered is highly personalized and may encompass advice on housing rights, accompanying individuals to appointments, and practical help for moving in, such as securing furniture and establishing payment plans for household expenses.
Ensure that individuals prepared to transition are provided with the security and support necessary to enhance their independence, dignity, and choices, aligning with the project's objectives.
Key Responsibilities:
- Offer service users information on housing alternatives, assist them in applying for accommodations, advocate on their behalf with housing providers, and accompany them to interviews and viewings when suitable.
- Serve as a key-worker for a designated caseload, empowering residents to take ownership of their journey with SAHA.
- Ensure clients receive essential practical assistance for moving in, either directly or through collaboration with other support services. This includes help with personal and housing benefit claims, community care grants, and domestic utility connections.
- Conduct a strengths-based assessment with applicants, facilitating the co-creation of a person-centered plan that enables individuals to achieve their aspirations and identify their talents.
- Develop and maintain effective partnerships with specialists and agencies that can provide resources, funding, or individual services relevant to the needs of service users, such as Registered Social Landlords, Local Authorities, Community Mental Health Services, Benefit Agencies, and other relevant organizations.
SAHA operates a supported accommodation service designed for single homeless individuals aged 16 and above. Clients come from diverse backgrounds and circumstances, including relationship breakdowns, rough sleeping, mental health challenges, and substance use. At SAHA, we strive to create a warm, respectful, and supportive environment for all clients, ensuring that everyone feels at home from the moment they arrive. Candidate Profile:
