Support and Care Coordinator

Role Overview: As a Learning Disability and Autism Specialist at Acuity Care Group Limited, you will play a vital role in supporting individuals with learning disabilities and autism to achieve their full potential. You will work as part of our Learning, Disability & Autism business unit to deliver high-quality care and support services.
Responsibilities: The successful candidate will have the opportunity to develop and implement person-centred care plans, work collaboratively with our multidisciplinary team, and coordinate care packages to meet the unique needs of each individual. You will also be responsible for monitoring progress and making adjustments as needed to ensure optimal outcomes.
Salary and Benefits: In addition to a competitive salary of £44,000, you will also receive a car allowance of £3,000 per annum. This is a permanent position that offers stability and career progression opportunities within our organisation.
