Complex Care Packages Coordinator

**Job Description**
We are looking for a Complex Care Packages Coordinator to join our team. In this role, you will be responsible for providing expert care to patients with complex needs, including those requiring tracheostomy and ventilator support.
As a key member of our team, you will travel across Lincolnshire to deliver field-based care, working closely with our clients and their families to ensure the best possible outcomes.
