Care Coordinator - Paediatrics

Location: Lincolnshire

Salary: £32,000-£38,000 per annum (£32 p/h days, £38 p/h nights)

We are seeking an experienced Care Coordinator to join our paediatric care team. As a Care Coordinator, you will be responsible for coordinating complex care packages for clients requiring high-acuity care.

This role involves working closely with our team of healthcare professionals to ensure seamless delivery of care. You will be required to maintain accurate records, communicate effectively with clients and their families, and work autonomously in complex situations.

Key Responsibilities:

  • Coordinate complex care packages for clients requiring high-acuity care
  • Maintain accurate and comprehensive records of patient interactions and care provided
  • Communicate effectively with clients, families, and healthcare professionals
  • Work autonomously and make sound decisions in complex situations
  • Develop and implement individualised care plans in collaboration with multidisciplinary teams

Requirements:

  • Valid Paediatric Nursing qualification and registration with the NMC
  • Experience working with complex care packages and high-acuity patients
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and make sound decisions
  • Strong commitment to delivering high-quality patient care
