Job Opportunity

We are seeking a talented and motivated Client Support Coordinator to join our team at Acuity Care Group Limited.

The successful candidate will work closely with our care teams to provide exceptional support services to individuals with learning disabilities and autism.

Salary: £41,000 per annum, plus a £3,000 car allowance.

Key Responsibilities:

  • Develop and maintain positive relationships with clients and their families.
  • Coordinate care plans and schedules to ensure seamless service delivery.
  • Maintain accurate records and reports on client progress and service delivery.

Requirements:

  • Degree in a relevant field or equivalent experience.
  • Proven track record in care coordination or administration roles.
  • Excellent communication and interpersonal skills.
