About this role:
- This is a full-time position based in the community, with up to 2 days working from home per week.
- The salary for this role is up to £45,000 per annum, depending on experience.
- You will have the opportunity to work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.
- This role will suit experienced RSCNs looking to further their career and join a stable, market-established provider.
Your responsibilities:
- Design and deliver a tailored care plan specific to an individual client’s needs.
- Effectively manage the MDT through strong communication and teamwork.
- To provide ongoing monitoring, support, and assessment of clients physical and physiological needs within your caseload.
- To deliver nursing interventions including the management of medicine delivery.
- Build and maintain sophisticated relationships with your clients’ family.
- Working closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.
