Care Coordination Specialist

This Care Coordination Specialist role offers a chance to improve the lives of individuals experiencing homelessness in East London.


The Job Description:
  • Deliver high-quality care coordination services to vulnerable individuals
  • Collaborate with colleagues to ensure seamless support services
  • Develop and implement effective support plans that cater to client needs
  • Maintain accurate records and reports

Salary Information:
  • £13.38 PAYE (inc hol) per hour equivalent, based on LTD company rate £15.70 per hour
  • 40 Hours Per Week, Early & Late Shifts on 7-day Rota
  • 12 Weeks, potential extension or perm
