Job Summary:

We are looking for an Advanced Palliative Care Specialist to join our Community Nursing Team, providing expert care to patients with complex needs.

Key Responsibilities:

  • Deliver high-quality palliative and end-of-life care to patients in their homes.
  • Supervise and mentor colleagues to ensure they have the necessary skills and knowledge to provide excellent patient care.
  • Collaborate with other healthcare professionals to develop and implement effective care plans.

Requirements:

  • Registered healthcare professional with NMC registration.
  • Post-graduate qualifications relevant to the area of practice, including non-medical prescribing and advanced clinical practice skills.
  • Evidence of continued professional development and implementation in practice.
  • Valid driving licence and access to a vehicle for community work.

Salary: £42,000 - £52,000 per annum depending on experience.
