Community Mental Health Specialist

Kent & Medway NHS & Social Care Partnership Trust is a leading provider of mental health services, and we are now recruiting for a talented Mental Health Specialist to join our team.
This rewarding role involves working as part of a multidisciplinary team to provide exceptional care to individuals with mental health conditions. As a Mental Health Specialist, you will have a key focus on promoting seamless and integrated care for patients with mental health conditions, supporting the transition of patients across care settings.
Your responsibilities will include supporting discharge planning to ensure a smooth and safe transition of care. You will also work closely with GPs, primary care pharmacists, and pharmacy technicians to deliver high-quality patient care.
The salary for this position is approximately £60,000 - £80,000 per annum, depending on experience.
