Community Nursing Team Lead

We are seeking a highly skilled and experienced Community Nurse to join our team as a Team Lead. As a key member of our leadership team, you will play a crucial role in the development and implementation of clinical strategies to improve patient outcomes.
About the RoleThe successful candidate will have a strong background in community nursing, with a minimum of 5 years' experience in managing complex caseloads and leading teams. You will be responsible for supporting junior staff, developing their skills and expertise, and contributing to the overall performance of the team.
You will work closely with patients, families, and carers to provide high-quality care and support, ensuring that they receive the best possible outcomes. Your excellent communication and interpersonal skills will enable you to build strong relationships with colleagues and stakeholders across the organization.
In this role, you will be expected to manage time effectively, prioritize tasks, and work under pressure to meet deadlines. You will also be required to maintain accurate records and participate in quality improvement initiatives.
Key Responsibilities- Lead and manage a team of community nurses to deliver high-quality patient care
- Develop and implement clinical strategies to improve patient outcomes
- Support junior staff in their professional development and mentorship
- Contribute to the overall performance of the team, including quality improvement initiatives
- Communicate effectively with patients, families, and carers to ensure seamless care delivery
To be successful in this role, you will need:
- A registered health care professional (NMC registration) with a health-related degree and evidence of relevant postgraduate development at Level 6
- Evidence of specialist qualifications relevant to area of practice (e.g., Specialist Practitioner District Nursing)
- A minimum of 5 years' experience in managing complex caseloads and leading teams
- Excellent communication and interpersonal skills
- Ability to manage time effectively, prioritize tasks, and work under pressure
