Job Opportunity

Cygnet Health Care is seeking a highly skilled and compassionate Person-Centered Care Coordinator to join our team. This is an exciting opportunity to work in a dynamic and supportive environment and make a real difference in the lives of individuals with mental health needs.

The ideal candidate will have a strong understanding of person-centered care principles and a commitment to empowering service users. You will work closely with our multidisciplinary team to develop and implement personalized care plans and promote independence.

Key responsibilities include:

  • Coordinating care and support services for service users
  • Developing and implementing individualized care plans
  • Collaborating with healthcare professionals to promote recovery and well-being
  • Maintaining accurate records and reporting incidents as required

We offer a competitive salary of £28,000 - £35,000 per annum, depending on experience, plus a range of benefits including 24-hour free GP support line and discounted gym membership.
