Job Description
We are seeking a Patient Care Coordinator to join our team at Leighton Hospital in Crewe. The successful candidate will be responsible for coordinating the care of patients across all ages, working closely with multidisciplinary teams to achieve optimal outcomes. This is an excellent opportunity for a motivated and dedicated nurse to develop their skills and experience in patient-centered care.
Key Responsibilities
* Coordinate the care of patients across all ages, including assessment, planning, and implementation of individualized care plans
* Collaborate with healthcare professionals to ensure seamless delivery of care
* Develop and maintain effective relationships with patients, families, and caregivers to promote self-management and independence
* Recognize and respond to urgent and emergency situations effectively
Requirements
* Registered Nurse with current registration with the NMC
* Proven experience in a hospital or community setting
* Strong communication and interpersonal skills
* Ability to work collaboratively as part of a multidisciplinary team
