About the Role
This full-time position, working 42 hours per week, involves providing personalized care and support to clients. As a Care Coordinator, you will work closely with a dedicated team to develop and implement tailored care plans that promote independence and well-being.

Key Responsibilities
  • Conduct thorough assessments to understand client needs and preferences.
  • Develop and maintain effective relationships with clients, families, and healthcare professionals.
  • Collaborate with the multidisciplinary team to create and update care plans.
  • Ensure timely and accurate reporting of client progress and any concerns.
