We are seeking an experienced Leaving Care Advisor to join our dynamic team at Service Care Solutions Ltd. This is an excellent opportunity to make a real difference in the lives of care leavers in Cheshire East.

Job Description:

In this rewarding role, you will be responsible for providing holistic support and guidance to care leavers, helping them prepare for independent living. Your expertise will be crucial in empowering young people to achieve their full potential.

Key Responsibilities:

  • Assessment and Planning: Conduct thorough assessments to identify care leavers' needs, developing personalized plans to address these requirements.
  • Support Coordination: Provide ongoing support and guidance, helping young people navigate the challenges of transitioning to independence.
  • Crisis Intervention: Respond effectively to crisis situations, adapting your approach to meet the individual needs of each young person.
  • Collaboration: Build strong relationships with local agencies and organizations, promoting a seamless transition for care leavers.

Requirements:

To succeed in this role, you will require:

  • Qualifications: NVQ Level 3 in Advice and Guidance, Health and Social Care, or equivalent education.
  • Knowledge & Skills: A deep understanding of young people's issues, assessment methods, and education/employment trends.
  • Other Requirements: Willingness to travel across Cheshire East and flexibility to work evenings and weekends when required.

Estimated Salary: £26,000 - £33,000 per annum, based on experience.
