
Leaving Care Case Manager

Join our Adult Social Care team as a Leaving Care Personal Advisor and make a real difference to the lives of care leavers in Cheshire East
In this key role, you will work closely with care leavers, their families, and other professionals to provide expert guidance and support. Your goal will be to empower young people to achieve their full potential by equipping them with the skills and knowledge they need to live independently and successfully navigate adulthood.
You will be responsible for conducting thorough assessments of care leavers' needs, developing personalized pathway plans, and providing flexible and responsive support in crisis situations. Your work may involve working outside of traditional office hours, but we offer a range of benefits and support to ensure that you can balance your work and personal life effectively.
To be successful in this role, you will need a strong understanding of young people's issues, assessment methods, and education/employment trends. You will also require excellent communication and interpersonal skills, as well as the ability to work collaboratively with others. A relevant qualification such as an NVQ Level 3 in Advice and Guidance or Health and Social Care is essential, as is a willingness to travel across Cheshire East and flexibility to work evenings and weekends when required.
