Community Dementia Support Specialist

We are seeking an experienced Outreach Worker to support individuals living with dementia in the Fife area. You will work closely with clients and their carers to establish personal goals and outcomes, providing guidance and assistance to help them achieve these.
The Outreach service is a short-term intervention offering support to people living with dementia and their carers, empowering them to live life to the full and as independently as possible.

Key Responsibilities:- Develop personal goals and outcomes with individuals living with dementia
- Provide ongoing support and guidance to help achieve these goals
- Reduce social isolation and build confidence in using public transport
- Engage with local communities and other stakeholders
- Relevant qualification and experience working with people with dementia
- Positive approach to dementia and willingness to participate in relevant training
