Dementia Outreach Programme Coordinator

We are looking for an experienced Coordinator to lead our Outreach programme, supporting individuals living with dementia in the Fife area. You will work closely with clients, their carers, and other stakeholders to establish personal goals and outcomes, providing guidance and assistance to help them achieve these.

The Outreach service is a vital part of our organisation's commitment to supporting people living with dementia and their carers. As a Coordinator, you will play a key role in delivering this service, ensuring that clients receive the support they need to live life to the full and as independently as possible.

  • Lead the delivery of our Outreach programme, ensuring that clients receive high-quality support
  • Work closely with clients, their carers, and other stakeholders to establish personal goals and outcomes
  • Reduce social isolation and build confidence in using public transport
  • Engage with local communities and other stakeholders
Requirements:
  • Relevant qualification and experience working with people with dementia
  • Positive approach to dementia and willingness to participate in relevant training
  • Flexible, inclusive, and creative approach
