As a Community Healthcare Professional, you will be an integral part of our team, working closely with patients, families, and other healthcare professionals to promote independence and optimal functioning.
Your key responsibilities will include:

- Conducting home visits to assess patients' needs and develop personalized treatment plans">
- Delivering evidence-based interventions to promote independence and prevent hospital admission">
- Collaborating with other healthcare professionals to ensure seamless care coordination">
- Maintaining accurate and timely records of patient progress and treatment outcomes">
- Participating in quality improvement initiatives to enhance patient care and service delivery">
We offer a competitive salary range of £38,890 - £44,041 per annum, depending on experience, and a supportive work environment that values teamwork, innovation, and continuous learning.
