About the Role:

We are seeking an experienced and dedicated individual to join our team as a Healthcare Professional for the Acute Frailty Unit within Ninewells Hospital.

The successful candidate will work collaboratively with a multi-disciplinary team to provide high-quality patient care, focusing on the rapid assessment and management of older adults with complex needs.

Key Responsibilities:

  • Deliver exceptional patient care, adhering to NHS Scotland's standards and guidelines.
  • Contribute to the development of individualized care plans, taking into account patients' medical history, social needs, and personal preferences.
  • Collaborate with healthcare professionals to ensure seamless transitions of care, promoting effective communication and coordination.
  • Participate in ongoing professional development, staying up-to-date with best practices in gerontology and frailty care.

Requirements:

  • Possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
  • Demonstrate a commitment to continued learning and professional growth, with a focus on medicine for the elderly.
  • Achieve the Mandatory Induction Standards For Healthcare Support Workers in Scotland and adhere to the Code of Conduct throughout employment.
