Job Title: Adults Mental Health Senior Practitioner

We are seeking an experienced and passionate Senior Mental Health Practitioner to join our Exmouth Core Adult Mental Health Team. As a key member of our team, you will play a vital role in providing expert assessment and treatment to individuals with complex mental health needs.

About the Role

As a Senior Mental Health Practitioner, you will be responsible for managing a defined and complex caseload, providing clinical supervision, and developing and maintaining therapeutic relationships with service users and their families. You will work closely with other disciplines and agencies to prevent hospital admission and promote recovery.

You will be passionate about making a difference to people's lives and will be committed to delivering high-quality care that is safe, effective, and focused on recovery.

Key Responsibilities
  • Providing complex assessment and care planning for individuals and their families/carers.
  • Engaging service users and carers assertively in the community.
  • Developing and maintaining therapeutic relationships with service users and their families and carers and other services to prevent hospital admission.
  • Providing proactive risk management approaches to help the service user and their family.
  • Developing and maintaining effective working relationships and networks with colleagues within mental health services, primary care, and other partner agencies.
Person Specification

We are looking for an individual with a professional mental health qualification, such as a RMN, Social Work Degree, or equivalent. You will have substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health problems.

You will be able to assess and develop plans to manage risk, plan creatively to maximise effectiveness of care plans, and have awareness of community structures and social care management responsibilities.
