Job Title: Senior Community Mental Health Practitioner

We are seeking an experienced Senior Community Mental Health Practitioner to join our Exmouth Core Adult Mental Health Team. As a key member of our team, you will be responsible for providing expert assessment and treatment to individuals with complex mental health needs.

About the Role

This is a substantive full-time post, 37.5 hours per week over 5 days. You will be working alongside a dynamic and motivated team to provide high-quality care to our service users. The successful candidate will have a professional mental health qualification and substantial experience of managing a complex caseload.

Key Responsibilities
  • Providing complex assessment and care planning for individuals and their families/carers.
  • Engaging service users and carers assertively in the community.
  • Developing and maintaining therapeutic relationships with service users and their families and carers.
  • Providing proactive risk management approaches to help the service user and their family.
  • Developing and maintaining effective working relationships and networks with colleagues within mental health services, primary care, and other partner agencies.
Person Specification

We are looking for a candidate with a professional mental health qualification, postgraduate qualification relevant to specialism, and training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff. The ideal candidate will have substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health problems.
