About the Role

The Workwell Programme Coordinator will support participants to start, stay, and succeed in work. This exciting opportunity is part of the Wyre Forest Health Partnership (WFHP) team, working closely with local services and the county-wide Work and Health coach network.

The successful candidate will work with people from diverse backgrounds, assessing barriers to returning to work and providing coaching, support, and health and employment advice to overcome these obstacles.

This role suits someone with experience in coaching and mentoring, as training will be provided for the successful candidate.

Main Duties

  • Manage a caseload of participants with common mental, physical, and social health problems or long-term conditions.
  • Carry out assessments to identify barriers and develop action plans in agreement with the individual.
  • Provide coaching and light-touch holistic support on a one-to-one basis to support the delivery of the participant's plan.
  • Onward refer to other services where additional support beyond one-to-one support is required.
