Job Description for Health and Wellbeing Coordinator

NHS Highland is looking for a skilled Health and Wellbeing Coordinator to join its team.

The successful candidate will be responsible for coordinating health and wellbeing services, conducting assessments, and developing care plans. The role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Main Responsibilities:

  • Coordinated health and wellbeing services to meet the needs of clients;
  • Conducted assessments and developed personalized care plans in consultation with multidisciplinary teams;
  • Maintained accurate records and reports as per NHS Highland policies;
  • Participated in regular supervision and training to enhance professional development.

Requirements:

  • Possession of a relevant qualification in social work or a related field;
  • Registration with the Scottish Social Services Council (SSSC) on the Social Worker part of the Register;
  • Experience working in a similar role, preferably in a healthcare setting;
  • Excellent communication and interpersonal skills.

Estimated Salary: £29,000 - £35,000 per annum, depending on experience.
